Using the attached assessment form, you are to answer all the question on the assignment sheet that is also attached. assignment is about the agency Green view Lodge Seniors care Resident (http://greenviewlodge.info) doing an annual MMSE (Mini Mental State Examination) on a client who’s name shall be left out for confidentiality reasons. The assignment is due Week 12.

 

The purpose of this assignment is to demonstrate effective skills in the gathering, analyzing, and synthesizing of data. The student may assess a client, a group, a program, or a social action issue and intervention.

 

The assignment is to be no longer than 4 single spaced pages. It is to be written in paragraph form.

 

The paper should include the following:

 

Assessments should include pertinent identifying data. This will differ depending on whether the assessment concerns an individual, group, or program. Some areas to consider are factual and relevant facts, such as, age, marital status, occupation, income to number of participants, criteria for membership, and so on.

 

There needs to be a statement of why the agency is involved, that is, who is the referral source or in the case of social advocacy, how did this particular issue come to the attention of the agency.

 

Describe the presenting issue or problem.   In terms of a group or program what is the mandate/mission of the program. Give details about the history of the problem or issue.

 

Further areas to consider in terms of individual assessments are physical and emotional functioning, relationships, especially as it pertains to family, educational and work history, and so on. It is necessary to include whatever data is pertinent to understanding the problem.

 

Other issues that need to be included are social environmental ones, such as the client’s level of income, where do they live, what impact does the neighbour hood have on causing the problem or in maintaining the problem. Social environmental issues are also important when assessing a group, program, or social action and factors to be explored include location of the agency, needs of community, and so on.

 

All assessments should reflect not only deficits and problems, but the strengths and resources of the client, group, project, or social action.

 

A major focus is the social service worker plan of action. What is the goal for intervention? What are the sub-goals? What have you and the client or participants agreed to work on for goal attainment? How will the intervention occur?

 

Strategies may include counselling, referral (be specific, where, when, why), advocacy for individual cases. Examples such as, consultation, procuring increased funding, hosting a meeting, hosting a workshop, changing participation rules, changing meeting times, may be considered for a group, project, or social action.

 

The assessment should include how the plan of action will be monitored. If some steps have already taken place they would be discussed.

 

The assessment will be marked holistically with special attention to the following:

 

  1. Identification of appropriate data
  2. Analytical skills/ Critical thinking skills
  3. Reliable and realistic goals are established
  4. A specific plan of action is documented which takes into account the strengths and resources of the client, group, program, or social action and where the interventions (helping   procedures) are realistic and appropriate
  5. Clarity and organizational skills

 

Marks will be deducted for the following:

  • Use of 1st. person or improper tense
  • Clinically or professionally inappropriate analyse
