Assignment (Case Study)

Your assignment for this module is to undertake the following tasks with the case study called “Introduction to the lab”

You are required to review the case study and undertake a cause and effect diagram to help to establish the root cause(s) of the problem.

You will then concentrate on one of the root causes and undertake a 5 whys analysis.

When completing the cause & effect and the 5 whys you will need to make some assumptions about the outcomes from your investigations. You need to CLEARLY articulate what these assumptions are.

The case study includes some references to ISO 15189, you need to review these clauses and articulate what they mean for the pathology laboratory.
