Case management is when skilled health care professionals are assigned to help individuals struggling with health concerns and don’t know how to manage their concerns, or where to get help. The case manager that is assigned to a particular case, meets with their client to assess their situation, determines options of intervention, helps the client set up services, evaluates effectiveness of interventions, and helps motivate the client to self-sustain (What is Case Management?, 2014). A case manager can be “a Registered Nurse, a Licensed Social Worker, or a vocational counselor, but cannot be a Licensed Practical Nurse, a community service professional working with the developmentally disabled” (para. 4 & 5). Case management can occur in numerous settings including a hospital where they help coordinate care to make sure interventions are efficient, thus minimizing gaps in care and saving payer sources unnecessary cost expenditures.
