Part B: Individual written report (35% of total weighting) Contribution to Learning This assessment will assist students to practise critical thinking and prioritisation of patients with complex medical/surgical needs identified from the assessment data. It will also assist students to develop a broader understanding on the importance of assessment and its implications in care planning and competent nursing practice. Assessment Task Students are to write a report detailing the planned care for the patient their group chose for Part A of the task. The written report should include the following elements:
– Identify all actual and potential health problems evident from assessing the patient data
– Choose two of the actual or potential health problems
-Identify the corresponding signs and symptoms of the health problem, as found in the case scenario
-Explain the pathophysiology associated with the given signs and symptoms Identify nursing interventions and provide evidence based rationales which will address the two chosen health problems
-Discuss the intended outcomes of the interventions identified
-Reflect on the potential effectiveness of these interventions including how you might measure the intended outcomes and what your plan would include if the outcome is not reached
