Leadership/Executive Board
•    What are the leadership/executive board roles?
•    Provide a description of the responsibility of each role.
•    Provide the name of the current person in that role.
•    What is the process in which members are chosen or nominated for the roles?
Committee/Task Force
•    What are the committees /task forces of the association?
•    Provide a description of the responsibility of the committee/task force.
Regional Associations
•    Does the state have separate regional associations? If yes, then provide a description of the associations.
Student Outreach
•    Does the association provide any educational opportunities/outreach for HIM students? This can be through scholarships.
Volunteer Opportunities
•    In what ways can a member volunteer with the association?
•    Contact the association, either through the Association President or a Committee Chair to identify a volunteer activity that you could complete for the association. An estimated amount of time for the activity is 3-4 hours.
