Project Guidelines
Purpose
The purpose of this assignment is to provide learners the opportunity to: a) analyze the epidemiology of the community, extrapolate healthcare priorities and plan a project to improve healthcare outcomes of the chosen community, b) design a population- focused project to decrease chronic disease and improve population health with measurable outcomes for evaluation purposes, c) develop interventional strategies based on primary, secondary, and tertiary levels of prevention, and e) integrate the project with the national goals and objectives of Healthy People 2020.
Course Outcomes
This assignment enables the student to meet the following course outcomes:
(CO#1) Synthesize knowledge and concepts from advanced practice nursing with supporting disciplines as a foundation for specialty nurse practitioner practice that is culturally competent and population-specific. (PO 7)
(CO#2) Advocate and negotiate for advanced practice policy to meet the healthcare needs of a culturally diverse society in a changing healthcare environment. (PO 10)
(CO#3) Contribute to the body of advanced practice nursing knowledge through participation in systematic inquiry, utilization of evidence-based practice, and dissemination of findings to support high-quality care and healthcare innovation. (PO 9)
(CO#4) Distinguish population-specific skills, understandings, and integrative abilities for genomic and primary healthcare screening of selected aggregate populations. (PO 11)
(CO#5) Analyze community resources related to abnormal screening results in selected aggregate populations. (PO 2)
(CO#6) Assess diverse populations within a community to ascertain health behaviors, interventions, and resources that affect health promotion and maintenance of selected aggregate populations. (PO 1)
(CO#7) Detail graphs and numerical summaries as descriptive statistical aids in determining a course of action relative to selected aggregate populations. (PO 7)
(CO#11) Generate hypotheses, equate appropriate statistical tests, assess the validity of the tests, and use test results to decide on a course of action relative to selected aggregate populations. (PO 7)

Requirements
Assignment Criteria for the Paper:
1. Search appropriate literature for “community assessment tools” For example: Lewenson & Truglio-Londrigan (2010) Public Health Nursing Assessment Tool.
2. Conduct a systematic community assessment, addressing all components (see table below) with objective, researched data. (This assessment should be conducted on the community in which the student lives (previously used for the Windshield survey).
3. An introduction to the community (1-2 pages) including demographics, population characteristics, epidemiology surveillance data.
4. Present analyzed aggregate data (presented in tables, graphs, charts, etc. as appropriate).
5. Choose one of the top-five health priorities that the assessment has identified and develop a plan for an interventional program/project to improve the health outcomes of the community/population
6. The program/project should include the priority health care issue selected, a design and Implementation strategy, a proposed timeline, and outcomes method for evaluation purposes.
7. The interventional program should be evidence-based.
8. The project should be integrated clearly with Healthy People 2020’s national goals and objectives.
9. The body of the scholarly paper is to be 10 pages in length, excluding title and reference pages.
10. Grammar, spelling, punctuation, references, and citations are consistent with formal academic writing and APA format as expressed in the 6th edition
11. Include a minimum of four scholarly resources published within the past 5 years, not including your textbook. References may include scholarly websites of organizations or government agencies and must be presented using APA format for electronic media.

Law enforcement, parks, playground, athletic fields, educational facilities, places of worship, social services, libraries, fire departments, shopping opportunities, facilities etc. (These are examples and not all inclusive)
Behavioral Considerations 25 5% Examples: Consumption patterns (nutrition, alcohol and illicit drugs, leisure pursuits and activities, willingness to engage in primary prevention strategies (These are examples and not all inclusive)
Healthcare Considerations 25 5% Examples: Existing health services (implementing primary, secondary and tertiary strategies), adequacy types of providers, health departments, availability, accessibility, culturally relevant, utilization, barriers, clinics, level of performance, alternative choices, health insurance, and attitudes (These are examples and not all inclusive)
Identification of high-priority issue based on analysis of data 25 5% Based on analysis of accrued data, what are the five priorities in the community that impact the health of this population. Choose one of the five priorities areas to use as the basis for an interventional program (the program is planned, but will not be implemented)
Project development 75 25% Develop an evidence-based, population-focused interventional program/project to address the priority issue. The interventions should use evidence-based research, have an established timeline, and include measurable outcomes that will be utilized for evaluation purposes.
