Paper, Order, or Assignment Requirements

I recently recived feedback from my professor with regard to my Concept Paper order. It turns out that the wrong template was used (my fault) I attach feedback from my professor to help guide the writer. I would be ok if the same writer is used to do the project. Also, the topic can remain the same.
