How would I create a change management plan which describes the process by which project scope changes will be initiated, approved/denied, communicated, tracked, etc.?

How would I create a change management plan which describes the process by which project scope changes will be initiated, approved/denied, communicated, tracked, etc.?

I am not looking for a communications plan; nor am I looking for someone to write my plan. (Just assistance in how to go about developing one.)
