FINAL PROJECT INSTRUCTIONS.

How to Tackle the Assignment

• Think about your organization, audience, and subject; why did you select this disease or indicator?

Community Profile

• Find the community you want to investigate.
• Before selecting your community, ensure that it has the necessary data available.
  • Example sites have been provided for you, including local health departments, on page 3.4.2.

• Remember to think, “If it is bad there, how bad is it compared to other communities?”
• When looking for the health issue, be sure to identify two health indicators that point to the health of the community.
• When possible, provide logic models or graphics that describe factors impacting progress toward optimal health.

Interventions

• Describe two existing program interventions for that community or for a similar population of interest.
  • Explain why you chose these two interventions.
  • What criteria or implicit criteria were used?
  • What evidence is available to show these were the most effective options?
  • Conduct SWOT analysis of local community health center.
• Provide a third intervention.
  • Include a budget and staffing plan to justify how this intervention works with respect to your logic model.

• Indicate how this will work.

Policy Memo

• Address your policy memo to the state, federal representative, or national government.
• Five components of a policy analysis:
  • Background, policy problem statement, landscape, policy options, conclusion
• Provide an explanation for your implicit and explicit criteria.
  • Implicit: What considerations went into your three to five options?
  • Explicit: What are the domains and measures you are using?
• Your recommendations

  • Include literature from peer review, gray literature, and white papers.
