In groups assigned by the professor you will design several health
record forms to submit as your project. The forms should include the
required data capture elements needed to provide the best possible
care to the patient. Remember that forms should always include the
revision date, form number, and name of the institution. Your team
creates the facility name and logo for use of the forms. The forms can
be formatted in Word or Excel. The forms should be applicable to
either paper or electronic formatted health records. The forms are to
be templates not completed forms. There is also to be a narrative
included to explain the process your team used to complete the
project- the strengths of the team and the perceived weaknesses of
what your team felt the team illustrated. Each team member will
complete the peer review assessment and submit it to the professor
before grades will be assigned to the project.
