What additional information will you need from our fictitious couple from last week’s Seminar (David and Angela), to help them begin to formulate a workable parenting plan for their children, Connie and Benjamin? What information will you need to divide property and debt? What information will you need to help the couple to formulate a post-divorce budgeting plan?

Develop a client intake form (you may choose to incorporate components of one of more forms developed by others with proper citation and/or attribution). This form is something that all clients will need to fill out prior to mediation. What information would you want to have as a mediator before you meet with your clients? What will help you to be prepared for your initial mediation session? Do not forget to start with the basics and get as detailed as you chose. Remember, this is your opportunity to ask all the questions you would like prior to mediation beginning.

TIPS: Your intake form should include material from all unit discussions, readings, and research, as well as from this unit. Make sure to include a reference page. This paper should be approximately 5-7 pages.
