GUIDELINES FOR SUBMITTING CLINICAL JOURNAL #2
Prior to submitting Journal 2, the student needs to have read Chapter 6 in the textbook. Journal 2 needs to include:

1. A community health nursing diagnosis using the format given in lecture and illustrated in Chapter 6. This diagnosis may be the one that will be used for the Community Health Policy Project or another diagnosis that has been identified with the PAR. (3 points) Format for writing community diagnosis given in the textbook and the power point must be for full credit to be given.

2. Summary of student clinical activities (be specific) since the submission of journal 1 up to the due date of journal 2. (Clinical activities must be population-focused) (2 points)

3.) Student clinical outcomes met to date (refer to student clinical outcomes for the course). The outcomes and the clinical activity that supports achievement of the outcomes must be written out. (2 point)

4. Documentation of course content as it relates to clinical activities. (Must document at least three areas of course content with explanation) (3 points)
