Overview

that is relevant to the themes of the module.

Proposed Structure

? Pathophysiology and related clinical manifestations – 500 words
Provide an overview of the underlying pathophysiology of your selected client’s condition and any relevant clinical manifestation. In order to demonstrate knowledge

and understanding, this discussion must be supported by a sustained use of literature and be clearly related to the selected client.
? Context of nursing assessment – 200 words
For this section, a clear and concise overview of the importance of a systematic approach to nursing care delivery is required. Additionally, some related discussion

on the selected model of nursing care would be beneficial.
? Formulation of care plan – 500 words.
For this section, you are required to formulate a nursing care plan using the template provided. The care plan must be based on Orem’s model of nursing. When

formulating the care plan, please select only ONE element from Universal self-care requisites and ONE element from Developmental self-care requisites OR Health-care

deviation self-care requisites.
NB: Please do not attempt to complete all elements of the care plan.
? Rationale for care delivery – 1300 words.
This section must provide the related rationale for the selected ONE element from Universal self-care requisites and ONE element from Developmental self-care

requisites OR Health-care deviation self-care requisites
covered in care plan above (650 words each).
This section must demonstrate an appreciation and use of related evidence to support content/selected actions within all selected stages of the care plan. Hence,

suitable content may include (but not limited to):
o Specific assessment tools;
o Mechanisms to support/optimise patient engagement/self-care;
o Nursing actions/evaluative strategies;
o Related multi-disciplinary involvement;
o Proposed method of care.
NB: For the section above, the marker must be able to see a sustained use of evidence to support the content of the care that been planned for the client.
