this is a case study.  focus on patient eduction only. interdisciplinary team is also part of the patient care. Who can he be referred to support his needs?and we need to have at lest 4 nursing diagnosis arranged according to the most important. the format will be a care plan (grid ) format.we may also need to write the Intro. and Conclusion in portrait and the care plan in landscape format .
