SOAP NOTE: The SOAP note must contain:

S — Subjective comprehensive health history – Information provided by the patient about the problem of interest as well as his/her general information regarding past medical/surgical/ trauma history, family history, medications, allergies, work, etc.

O — Objective data – Including ALL physical assessment findings related to the disorder Asthma, lab results, diagnostic procedures, etc. You must explain the rationale for any normal and abnormal results and how the correlate with the disorder. Example: glycosylated hemoglobin was used to determine compliance with diabetic control. The client’s result of 6 indicated compliance for the last 120 days.

A — Assessment – Complete, comprehensive assessment/diagnosis list including the disorder under study (Include all diagnosis present, not just the ones that are related to the asthma). Please use medical terminology when referring to diagnosis, disease process and other related disorders. If the client is taking medications, please list the name and rationale for use.
