Performance evaluation

I’m having a hard time filling out this performance evaluation and I’m in need of help. I will include some information such as my resume, personal essay written by Advanced Writers, reference letter and some detalied information in regards to my nursing career. I want at least two examples for each question please.

Here is a list of things I have done:

-I have worked in a variety of positions as a Psychiatric Nurse, and I continue to stay on top of my skills through education (currently working on MSN degree for Family Psychiatric Nurse NP), continuing education courses, internet, and nursing journals. Also, I share information with co-workers based on evidenced based practice. I make sure that I’m familar with new information to provide the highest quality of patient care. Also, I research JCAHO Behavioral Health Care: 2015 National Patient Safety Goals to provide high quality patient care. I do alot of patient/family teaching in regards to treatment and medication. I provide hand-outs if needed. I’m a team player I make sure that everyone is involved in the patient’s treatment from the patient, family, therapist, doctor, nurse, and mental health tech. In a previous job I did help with a staffing issue that was making the work environment unsafe. With a group of other nurse’s we came up with self scheduling to help benefit the unit to decrease the safety concerns. We used evidenced based practice research to help us with this project.

I was not able to scan in these references from previous co-workers so I will take some sentences from each one to give you an idea about me:
-Comora worked well under stress and demonstrated maturity when working with both adult and pediatric patients with special physical and emotional needs.
-She worked very well with familes who were in crisis and provided a supportive and nursing level of care to her patients.
-She effectively cared for patients with extensive needs who other nurses would have difficulty providing an appropriate, non-judgmental level of care.
