Please see the decryption word documents. I organized database and did the indexing, so you do not need to do that.

First as you can see pages in excel, we had raw data. 

First i added last 3 criteria as she asked for additional point (which is 65 and older, death rate, killed in accidents) , than i converted all data to same unit to percentage and than i run the correlation and than i converted positive units such as non-smokers to smoker and than I sum all those negative percentages and I calculated index. I sort them from highest to lowest. I run descriptive statistic. So you need to write all these thing in word document. 

So from the index page you can see the highest point is the worst state. 

You need to write a report about this analyzes in a word document with references. I attached a sample report in pdf. It should be exactly like that report. You need to group all index number in 4 category good, moderate, poor, and bad with numbers. You have to use social significant index as pdf file explain what is it and how they did. ocial significant index is you need to find the highest and lowest and divide it to 4 and it will give you 4 different category. highest is the worst lowest is the good. You need to give all the numbers and methodology in word document.
