Value/Belief Pattern
1.    What are the predominant ethnic and cultural groups along with beliefs related to health?
Hispanic/Mexican of Catholic or Christian faith
The most common definition of health for this community was described as the activities they participate in to maintain good health. Such activities included going to the doctor for regular check-ups as well as when feeling sick, exercising on a regular basis, eating healthy, no tobacco use, minimal to no alcohol ingestion, and taking all of their prescribed medications for any acute or chronic illnesses.

2.    What is the availability of spiritual resources within or near the community (churches/chapels, synagogues, chaplains, Bible studies, sacraments)?
Woodlawn Christian Church, Woodlawn Lake Methodist Church, St. Ann’s Catholic Church, University Park Baptist Church

3.    What do the community members spend their money on? Are funds adequate?
Groceries, eating out, clothes, school supplies for themselves or their children, and entertainment.
Their funds are not usually adequate, but they find a way to manage or use a credit card.
Health Perception/Management
1.    What are some predominant health problems? Compare at least one health problem to a credible statistic (CDC, county, or state).
Type 2 Diabetes, Obesity, and High Blood Pressure (Women with Type 2 Diabetes are at risk for high-risk pregnancies).
Type 2 Diabetes: Woodlawn Lake is in Bexar County whose prevalence according to the City of San Antonio Metropolitan Health District is 13% of the total population and within: White 8%, African American: 12%, Hispanic 16% compared to these rates: USA 9% and TX 10% (COSAMHD, 2015).

2.    What health professionals and health resources are available within the community, and usage?
The City of San Antonio Metropolitan Health District (MHD) is located very close to the community of Woodlawn Lake. The MHD has offices, which offer immunizations, and large amounts of preventative information regarding health living, Diabetes prevention, healthy babies, healthy homes, etc. both online an in person (COSAMHD, 2015).

3.    What prevention programs (i.e. health and/or fitness) are available? Does the community think these are sufficient?
The YMCA offers a Diabetes Prevention Program to assist those with a high-risk of developing Type 2 Diabetes adopt and maintain a healthy lifestyle to reduce their chances of developing the disease. The MHD offers FREE workshops for those with Type 2 Diabetes and their families (COSAMHD, 2015).
Yes, the community does feel that what is offered is sufficient. These are just a couple of examples of preventative programs available. As the MHD and other organizations offer an array of services for those who desire to partake for the betterment of their health.
Nutrition/Metabolic
1.    Are there any nutritional programs available to mentor expecting mothers about eating healthy?
Yes, WIC has recently opened the first Baby Café, which provides pregnant women (and breastfeeding mothers) access to resources such as nutritionists and registered dieticians in a casual environment. WIC clinics also provide nutrition education via online classes and in person (COSAMHD, 2015).

2.    Are there any available sources (VIC, food boxes, soup kitchens, meals- on- wheels, food stamps) to provide healthy meals for poor or single mothers that are expecting?
WIC benefits/assistance is available for expectant and women with children alike who meet the financial criteria. Once approved they will receive a debit-like card to purchase groceries, encouraging fresh and healthy food purchases.

3.    Obesity is becoming a growing concern for women of childbearing age. What is the percentage of obese pregnant women in this clinic?
Overweight: 47% (BMI: 25.0-29.9)
Obese: 14% (BMI: 30+)
Elimination/Environmental Health Concerns
1.    Traffic pollution may be a risk while pregnant. Are there any cases of newborns with lung problems?
Yes, but without relation to traffic pollution or any kind of pollution and is mostly related to a virus, bacteria, or a congenital malformation.

2.    Does this clinic have any outside sources to inspect living condition of expecting mothers? For example if they have access to clean bathrooms, supplies and laundry services?
No they do not and not without very good reason. Not other than utilizing their facility’s (University Health System) social and case workers and involving CPS (Child Protective Services).

3.    A congenital infection is an infection that crosses the placenta to infect the fetus. How many cases of infectious disease are observed monthly/ yearly related to poor hygiene?
None in relation to poor hygiene, but sometimes related to an STD or IV drug use exposure.
Activity/Exercise
1.    What kind of recreational facilities does the community provide for its residents and is it easily accessible for all?
Woodlawn Lake Park is in the center of this community and offers multiple fitness opportunities: track with exercise station, large playground, softball field, pool, tennis courts, gym with indoor basketball courts. Within the gym there is also a Community Health & Wellness Center, which offers fitness classes for all ages. Yes, it is as it is within walking/jogging/cycling distance for many and the bus has several stops near the area as well.
2.    What public transportations are available to assist the residents of this community to get to these programs and recreational facilities?
The VIA Metropolitan Transit bus system is available offering student, senior citizen, and frequent rider discount opportunities.
3.    What kind of activities does the community do together as a whole?
Woodlawn Lake Park is at the center of this community and offers an array of community gathering opportunities year around. Such as an Easter Egg Hunt, 4th of July Fireworks show, Fiesta related events as well as fitness events such as basketball and softball games/tournaments.
Sleep/Rest
1.    How many hours of sleep do the residents get per night?
Clinic population average: 6-8 hours/night during the week and 7-9 hours/night on the weekends
Community average: 5-6 hours/night during the week and 7-10 hours/night on the weekends
2.    Do they use sleep aids to help them fall to sleep?
Clinic population: on average most did not and those who did take something recommended by their doctor or use a home remedy such as warm milk.
Community population: Most also said that they did not, they are usually very tired and have no troubles falling asleep, but if they need something they usually take an OTC product.
3.    Do the residents of this community consume a lot of caffeinated products?
Clinic population: Most said that they try not to, however if they do they limit it to 1-2 beverages a day (coffee or soda)
Community population: Average answer was 1-2 beverages a day for adults and children
Cognitive/Perceptual

1.    What is the predominate language of this community, any communication barriers?
English with Spanish a close second due to many in the community are bilingual.

2.    What is the education level of this population?
Community: HS graduate: ~18.6%
College w/ no diploma: ~11.4%
Associate’s or Bachelor’s: ~ 3.3%
Master’s or higher: ~1.1%

3.    With reference to access to Internet and the library, what resources are available to this community and how is the community using it?
There are at least 2 public libraries (Main Downtown Library and Bazan Library), and the Woodlawn Lake Community Room, which all offer free Internet access. There is also San Antonio Community College nearby for those who are active students. Most stated they use the Internet for research, applying for jobs, and personal use.
