Personal Wellness Plan

General Guidelines: Develop and write a personal wellness plan which targets a selected health behavior goal. Review your wellness profile to help you determine the health behavior on which to develop your plan. Choose a behavior that is important to you and that you feel motivated to change.
Introduction:
° Include a statement of your health behavior goal. You goal should be clear, specific, measureable, and attainable. For example, your goal could be to incorporate I hour of physical activity a day, five days a week.
° Include an explanation of the reason you decided to select the specific health behavior goal. For example, the results of your cardiovascular fitness test increased your desire and motivation to become more physically active.
Body:
° Identify and discuss at least three health benefits associated with your goal. For example, incorporating more physical activity into a daily lifestyle can substantially increase your energy level and stamina and reduce the risk for heart disease.
° Identify and discuss at least three negative health consequences that may result if action is not taken toward the goal. For example, an inactive lifestyle can increase disease.
° Explain in detail three strategies you will implement to achieve your health behavior goal. For example, for the first three weeks of your plan, you will walk for 15 minutes on Monday, Wednesday, and Friday during your lunch break. You will plan to gradually increase the number of minutes you spend and the number of days a week you will walk.
° Identify in detail three obstacles that may need to be confronted as move toward your goal. Discuss possible solutions in dealing with each obstacle. For example, lack of time might be a barrier for becoming more physically active. A solution might be evaluating a typical day and determining when you can fit in additional physical activity.
Conclusion:
°Discuss the benefits you will receive once you have achieved you goal. For example, you will have more energy at the end of the day to do the things you enjoy.
° Indicates how you will reward yourself for achieving your goal. For example, you will reward yourself with a new music CD.
